Commercial drivers are required by the Federal Motor Carrier Safety Administration (FMCSA) to complete a Department of Transportation (DOT) physical exam to ensure they are medically fit to safely operate a commercial vehicle. How to Prepare for Your Appointment
To help your exam go smoothly, please bring:
- A valid driver’s license or photo ID
- A list of current medications, including dosages
- Your corrective lenses (glasses or contacts) if required for driving
- CPAP compliance report (30-day printout) if you have sleep apnea
- Medical documentation for any existing health conditions (e.g., diabetes, heart disease, hypertension)
- Avoid smoking or vaping at least 4 hours before your exam to help maintain optimal blood pressure readings
- Do not urinate immediately before arrival — a urine sample will be collected on site
What to Expect During Your Exam
Your certified medical examiner will conduct a comprehensive health assessment to ensure you meet federal safety standards. The evaluation includes:
- Vision and Hearing:
Checking that your eyesight and hearing meet FMCSA standards, including visual acuity, peripheral vision, and the ability to detect a forced whisper. - Cardiovascular and Heart Health:
Listening to your heart, reviewing your history, and assessing for any cardiovascular conditions that could affect safe driving. - Blood Pressure and Pulse:
Measuring your blood pressure and heart rate to confirm they’re within a safe range for certification. - Musculoskeletal System:
Assessing your strength, balance, and joint mobility to ensure you can safely perform driving-related tasks. - Neurological and General Physical Exam:
Evaluating reflexes, coordination, and general health for any conditions that could interfere with safe vehicle operation. - Urinalysis:
A standard urine test is performed to screen for blood, sugar, and protein levels—indicators of underlying medical conditions such as diabetes or kidney disease. (Note: This is not a urine drug screen.)

If you meet all requirements, you’ll receive your Medical Examiner’s Certificate (MCSA-5876) the same day, valid for up to two years. If follow-up is needed, our team will guide you through the next steps to help you stay compliant and on the road.
